At Hebe Care we put the service user front and centre of our assessment process. We view it as a two-way process, taking into account the needs of the person involved and our ability to provide the correct level of care for them to flourish.
Get a Response in 24hrs
Our lead assessor will respond to any referrals within 24 hours to start an initial conversation around the suitability. If appropriate we speak to the service user, their immediate family, and current support team. We assess the level of care required and may request supporting information, such as risk assessments, MDT reports, positive behaviour support plans, and incident reports. This rigorous approach enables us to build a plan in-house with our development team and, where required, our clinical team.
Work with us
Once the information has been gathered and assessed, we will conclude whether or not a placement can be offered. If one can be offered, we will work with the user, their family, and support team to streamline the process, ensuring it is done sympathetically and carefully. In the instances where care cannot be offered we will communicate with the referrer, offering a full explanation as to why the user’s needs cannot be met on that occasion.
Start the Process
Following a successful assessment we submit the proposal, detailing the levels of support required, written assessment, cost breakdown, and any additional input from our internal team. A commissioner will review the proposal and decide whether or not the placement can be funded. Once agreed, we can start the process of the move, making the transition into supported care as efficient and stress-free as possible.
